Severity (info/low/medium/high/critical) sets urgency within warn and fail. “As of” marks a periodic snapshot (Brain RLS, repo audit); refresh it before acting since it may predate a fix. Checks with no “as of” are live at scan time.
Accepted by design items are intentional and need no fix; the rationale is shown inline.
Responding: decide genuine gap vs accepted-by-design. Fixing a real gap usually touches RLS, grants, auth, or secrets, so it runs the Tier 2 flow (claim, security review, independent second-session countersign, apply, log, release); queue a remediation session rather than editing directly. Re-run the scan afterward to confirm it flips to pass. An RLS-coverage fail is drop-everything: run_dashboard_query is SECURITY INVOKER, so table RLS is the dashboard’s enforcement boundary.
Live DB checks recompute on demand; snapshot items refresh out of band. The weekly scan runs Mondays 08:30 UTC; “Run scan now” forces a fresh run. “Export details” on any finding downloads a Markdown brief; “Export report” downloads the full set.
